    possible values:
    (+) farsightedness              (-) nearsightedness
    PLANO or PL = nothing, enter 0.00 in the form
 
  Cylinder & Axis, astigmatism and associated angle, they come in pair
    possible values:
     (+)or (-) power, and associated axis
    DS/SPH/Sphere = nothing, enter 0.00 in cylinder and 000 in axis
 
  OD = right eye  OS = left eye
 
  ADD power added for near vision in bifocal and progressive lenses
    possible value:
    (+) power
